Families From Calgary and Area​

Families In Southern Alberta
Families that are unable to make it to Calgary for an outfitting can fill out a Remote Outfitting Form. These families should submit this with their application. When filling out the Remote Outfitting Form and Application please refer to the Equipment Measurement Legend for the required measurements for our most popular sports. If you need assistance in measuring please refer to these videos. Please submit your completed Remote Outfitting Form to admin@flamessportsbank.ca, by fax at 403-202-1961, or by mail at 3557 52nd Street SE Calgary, AB T2B 3R3.
Agencies, schools & Registered Canadian Charities
Agencies, schools and Registered Canadian Charities (qualified donees) that are requesting equipment with the Group Application for Equipment form can use the Remote Outfitting Form and Equipment Measurement Legend when requesting equipment for individual kids. If you need assistance in measuring please refer to these videos.
